  1. Step 1 of 5: See if you qualify

    Work hours and income matter

    • Kids under 13 (under 18 special needs)
    • Single parents: 15+ hours a week
    • Family of 3: up to $6,637 a month
    Good to know: Social Security numbers and immigration status info are NOT required for child care eligibility.
  2. Step 2 of 5: Apply your way

    Online, in person, or by phone

    Good to know: Phone help is available in many languages.
  3. Step 3 of 5: Send your papers in 30 days

    A worker sends you a checklist

    • Kids' birth certificates
    • Recent pay stubs
    • Work schedule from your boss
    • Utah ID or driver's license
    Don't miss: Send all papers within 30 days or your application is denied. After 60 days you must start over.
  4. Step 4 of 5: Pick your provider

    Important: Family, friend, or neighbor caregivers must get DWS-FFN approval and pass background checks before they can start.
  5. Step 5 of 5: Get your answer and start

    • Approval or denial notice arrives
    • State pays provider every month
    • You pay a co-payment
    Important: Report changes within 10 days. If your provider charges more than the state rate, you pay the difference plus your co-payment.
    Good to know: If you lose your job, tell them right away — you may keep benefits while you look for work.
