    • Babies and toddlers birth to 3
    • Delays or disabilities
  2. Step 2 of 5: Make a referral

    Good to know: Your pediatrician can start the referral for you.
  3. Step 3 of 5: Get your papers ready

    • Birth certificate
    • Utility bill or lease
    • Doctor's info and records
    • Notes on milestones
  4. Step 4 of 5: Get an evaluation

    • Full development check
    • Within 45 days
  5. Step 5 of 5: Start services

    • Family Support Plan (IFSP)
    • Therapy and family support
    Good to know: Services continue until age 3 or until your child's goals are met.
    Good to know: If your child is not eligible, the team can point you to other community help.
