    • Child is 3 to 5

  2. Step 2 of 5: Get your papers ready

    Gather these first

    • Child's birth certificate
    • Proof you live in Chicago
    • Proof of your money
    • Shot records and check-up
    Important: Make sure papers are clear and up to date to avoid delays.
